Guyon canal syndrome is a compression neuropathy of the ulnar nerve at the wrist level. The ulnar nerve runs through a narrow anatomical canal on the little-finger side, called the Guyon canal. When it is compressed, sensory disturbances, weakening of the hand muscles or both symptoms together appear. The nature of the symptoms depends on the site of compression and on whether the sensory part, motor part or the whole nerve is affected.

Causes of Guyon canal syndrome

The condition may be caused by chronic pressure on the wrist area, injury, post-traumatic changes or the presence of structures compressing the nerve. In some patients the source of the problem is a ganglion cyst that presses on the ulnar nerve. Symptoms also occur more often in people performing repetitive wrist movements, using vibrating tools for long periods or practising sports involving prolonged support on the palms, such as cycling.

Symptoms can develop gradually and may be downplayed at first. Over time, however, they make daily functioning and simple tasks requiring an effective grip more difficult.

Symptoms of Guyon canal syndrome

Patients most often report numbness, tingling or burning of the little finger and part of the ring finger. Characteristic features also include weakening of hand strength, problems holding objects and difficulty performing precise tasks such as writing, fastening buttons or using a keyboard. In more advanced cases the hand muscles may waste away and the fingers may take on a characteristic position called "ulnar claw".

In contrast to carpal tunnel syndrome, symptoms mainly involve the little finger and the ulnar part of the ring finger rather than the thumb or index finger.

What does diagnostics look like at OpenMed?

Diagnosis begins with a detailed interview and a physical examination. Our doctors assess the location of symptoms, the strength of the hand muscles and the function of the ulnar nerve. Clinical tests that help determine the site of nerve compression are also performed during the examination.

To confirm the diagnosis, additional tests may be ordered, such as electromyography (EMG) and nerve conduction studies, which allow assessment of ulnar nerve function. In selected cases, imaging studies including wrist ultrasound or magnetic resonance imaging are also helpful, making it possible to detect cysts, post-traumatic changes or other structures causing compression.

Treatment of Guyon canal syndrome

The method of treatment depends on the cause and the severity of symptoms. In the initial stages, conservative treatment is often possible. It includes limiting activities that intensify symptoms, modifying occupational or sporting activity, using stabilisers and rehabilitation aimed at improving nerve function and reducing overload of the wrist.

If the cause of compression is a cyst or other anatomical change, or if symptoms persist despite conservative treatment, the doctor may discuss the option of surgical treatment in an appropriately chosen centre. Early diagnosis increases the chance of complete resolution of symptoms and reduces the risk of permanent nerve damage.
